Influence of HER2 Overexpression on Response to Cetuximab in Patients with Metastatic Colorectal Cancer, Tehran, Iran

Objective: Colorectal cancer (CRC) remains the fourth cause of mortality in the world. HER2, an epidermal growth factor receptor (EGFR) has an influence on prognosis and overall survival in breast and metastatic gastric cancers. We evaluated the prevalence of HER2 positivity among Iranian patients with CRC and determined the response rate to Cetuximab, an EGFR-targeted drug in eligible HER2 positive patients.
Methods: HER2 immunohistochemistry staining was carried out in samples of 150 CRC patients and the prevalence of HER2 overexpression was estimated. We also investigated the association of HER2 overexpression with the prognostic factors, overall survival (OS), and progression-free survival (PFS) as well as response to Cetuximab in the patients by reviewing the medical records.
Results: The prevalence of HER2 overexpression was12%. The mean of follow-ups was 40.67 ± 21.57 weeks in the patients with HER2 overexpression. PFS and OS were lower in HER2 overexpressed patient than other. There was only a borderline relation between the distant metastasis and HER2 expression. Considering the response to Cetuximab, there was a significant difference between HER2 positive and HER2 negative groups.
Conclusions: Our study provides further evidence in the effect of HER2 overexpression in CRC patients on the OS, PFS, and response to Cetuximab.

Technical Modification of Self-expanding Metallic Tracheobronchial Y-shaped Stents Implantation in Treatment of Malignant Tracheal Carina Stenosis Hot!

Show Abstract
Objective: To assess the technical feasibility and effectiveness of fluoroscopically-guided self-expanding metallic tracheobronchial Y-shaped stents implantation under local anaesthesia by introducing two stiff guidewire at one time for shortening the procedure time.
Methods: A total of 16 self-expanding metallic tracheobronchial Y-shaped stents implantation procedures were retrospectively enrolled in this study. In 6 procedures the two stiff guidewire were routinely introduced respectively. In another 13 procedures the two stiff guidewire were introduced at one time. The technical success rate, average procedure time was documented.
Results: 19 self-expanding metallic tracheobronchial Y stents were all successfully implanted in treatment of malignant tracheal carina stenosis. The average procedure time of routine group was 15.7 mins, while the technique modification group was 11.0 mins. The difference between the two was significant (P = 0.001,2).
Conclusion: The two stiff guidewire introduced at one time can shorten the procedure time significantly in self-expanding metallic tracheobronchial Y stents implantation.

Effects of Life Skills and Twelve Steps on Problem Solving and Risk-taking Behaviors among People Living with HIV, Tehran, Iran Hot!

Objective: The present study aimed to compare social problem solving ability and risk-taking behaviors among people living with human immunodeficiency virus (HIV) (PLWH) and healthy individuals in Tehran, Iran, 2013.
Methods: In the study, 50 patients aged 20 to 45 years living with HIV were assessed in Valfajr health Center in Tehran. Moreover, 50 healthy individuals considered as the control group. To evaluate the ability to solve a social problem, standard questionnaires were used for social problem solving and risk-taking assessment.
Results: The results showed that there was no difference in social problem solving ability and risk-taking behaviors between the PLWH and the healthy individuals. Moreover, there was no difference in all subscales of social problem solving ability (positive problem orientation, negative problem orientation, rational problem solving, impulsivity/carelessness and avoidant style). Furthermore, there was not seen any differences in the rate of risk taking between the two groups.
Conclusion: Because of passing life skills and twelve steps, PLWH in social problem solving use positive problem orientation and rational problem solving exactly like healthy individuals. Furthermore, among these population, the rate of risk-taking is similar to healthy people.

Endobronchial Ultrasound-Guided Transbronchial Needle Aspiration (EBUS-TBNA) in Mediastinal Lesions

Show Abstract
Objective: Mediastinal lesions can be difficult for sampling to diagnose. The purpose of this study was to investigate the application value of endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) in the diagnosis of mediastinal lesions.
Methods: Between January 2013 and June 2017, 1,020 cases were screened to perform EBUS-TBNA at our institutions. All cases were provenmediastinal masses through computed tomography (CT) or positron emissiontomography (PET)/CT examinations.
Results: 1,020 patients underwent EBUS-TBNA and 2,142 lymph nodes punctured. 817 (80.1%) cases wereconfirmed as malignancy; 169 (16.6%) cases were identified as benign. In theremaining patients, twenty-six patientswere examined as malignancyby mediastinoscopy or thoracoscopy. The sensitivity, specificity, and accuracy value of EBUS-TBNA per patient were 95.8% (817/852), 100% (169/169), and 96.5% (984/1020), respectively. All patients werewell-tolerated with no serious
complications, only 32 minor complications were recorded.
Conclusion: EBUS-TBNA is a safe and sensitive modality for the histopathological diagnosis of mediastinal lesions.

Relation between Personality Dimensions, Risk Perception and Coping Strategies in People Living with HIV

Show Abstract
Objective: This study was performed with the aim of determining the relationship between personality dimensions, risk perception and coping strategies in people living with HIV (PLWH) in a Voluntary Counseling and Testing (VCT) center in Imam Khomeini Hospital and Karaj in 2016.
Methods: The research sample size consisted of 100 (52 female and 48 male) PLWH who visited the VCT center and completed short neo-form tests, the Bentin risk perception, and the Lazarus-Folkman coping strategies test.
Results: Problem-focused coping strategy had a positive correlation with extroversion personality dimensions (r = 0.46; P= 0.01), agreeableness (r = 0.36; P = 0.01), and conscientiousness (r = 0.39; P = 0.01). Moreover, risk perception showed a negative correlation with neuroticism (r = -0.28; P = 0.01), while it showed a positive significant correlation with extroversion (r = 0.30; P = 0.01), agreeableness (r = 0.40; P = 0.01), and conscientiousness (r = 0.34; P = 0.01). There was a statistically
significant relationship between risk perception and personality dimensions (P < 0.001). Multivariate regression indicated that neuroticism (P = 0.03), agreeableness (P = 0.01), and conscientiousness (P = 0.03) had the ability to predict risk perception, while extraversion (P = 0.01) was able to predict problem-focused strategies.
Conclusion: Based on our findings, other factors apart from improving the level of information are involved in this disease. Thus, information regarding the mental spect of PLWH may find the missing link in this regard alongside treatment. Accordingly, the result obtained from this research can be used in preventive programs for PLWH.

Peripheral T-cell lymphomas (PTCLs) are a group of heterogeneous lymphoproliferative diseases arising from the mature T cells derived from posterior thymus. Five subtypes account for most of the PTCLs diagnosed worldwide, that is, T-cell lymphoma-not otherwise specified (PTCL-NOS), angioimmunoblastic T-cell lymphoma (AITL), ALK-positive anaplastic large cell lymphoma (ALK-positive ALCL), ALK-negative anaplastic large cell lymphoma (ALK-negative ALCL), as well as enteropathy
associated T-cell lymphoma (EATL). These cancers usually need immediate systemic chemotherapy, however, poor prognosis always occurs. PTCL has significant heterogeneity in clinical, pathological and molecular levels. Genomics has been identified as a powerful tool for defining biology and molecular features of tumors. Genomic technologies, including microarray technology and next-generation sequencing technology, have enabled a better understanding of the molecular bases of these cancers, help to refine the classification of PTCLs and are beneficial for individualized therapy. Lots of drugs approved by Food and Drug Administration (FDA) directed towards gene mutants and the signaling pathways suggest the usefulness of whole genome techniques to identify therapeutic targets. In this review, we will describe the molecular signatures and genetic findings of PTCLs and discuss new therapeutic opportunities derived from these findings.

Show Abstract
Systemic mastocytosis (SM) is a hematopoietic stem cells neoplasm that presents a range of asymptomatic indolent course to an aggressive fatal condition. Acute mast cell leukemia (aMCL) is a rare rapidly progressive SM variant which traditionally was treated with tyrosine kinase inhibitors such as imatinib due to the involved KIT pathway in the pathogenesis of aMCL with unsatisfactory results. Therefore, polychemotherapy (Poly-CT) and allogeneic hematopoietic stem cell transplantation (HSCT) is now recommended.
We report a case of primary leukemic aMCL presented with bilateral breast masses, without response to imatinib despite no common KIT mutations. we suggest Poly-CT, and HSCT for optimal management as soon as possible.
