Concerning Unprotected Sex and Illicit Substance Use among Iranian University Students Hot!

Show Abstract
The university student population is the largest adolescent group in Iran. we aimed to measure the prevalence of sexual and illicit substance use risk behaviours among university students in Tehran, Iran. This study employed a quantitative method strategy to study Iranian university students. An anonymous quantitative survey was conducted using a self-administrated questionnaire. About 392/400 (98.0%) of invited students completed the survey with an average age of 21.98 (18-35), and 58.7% male. Lifetime substance use was reported by 14.8% of participants. The most commonly used substances were grass (11 .0%), hallucinogens (4.1%), and cocaine (2.6%). Moreover, 36.7% of participants reported sexual contacts within the last 12 months. Compared to females, more males reported condom use in last sexual contact (30.7% vs. 10.0%, P < 0.005), and multiple sexual partners (40.6% vs. 16.7%, P < 0.005). About, 34 (27.2%) participant reported sex with a sex worker or a person who had a multiple sexual partner. Group sex was reported by 5.1% of participants. Illicit substance use and unprotected sex are considerably high among university students. More effective prevention programs accessible to them are needed to reduce their vulnerability to associated harms to such risk behaviours.

Efficiency of Magnesium Sulfate versus Succinylcholine in Rapid Sequence Intubation Hot!

Background: Rapid sequence intubation (RSI) is an advanced airway management medical procedure used to achieve tracheal intubation under general anesthesia in patients who are at high risk of pulmonary aspiration. Succinylcholine is a common medication used during RSI with paralytic aim. On the other hand, Magnesium sulfate also has muscular relaxant effect. The aim of this study is the efficiency of magnesium sulfate instead of succinylcholine in RSI process will be studied. Methods: This single blind clinical trial contained 90 patients divided into 2 equal groups. Group 1 received succinylcholine based on usual process of RSI and Group 2 received magnesium sulfate at preoxygenation step of RSI without succinylcholine injection. Heart rate, systolic and diastolic blood pressure for each patient had been measured at 6 different times. Obtained results were analyzed by SPSS ver. 22. Results: The results showed significant reduction of the heart rate in all patients (P < 0.05). The heart rate of both groups at corresponding time were also compared and it had significant differences in most of critical times. These results also repeated for systolic and diastolic blood pressure. The excellent acceptability and quality of intubation in group 1 was higher rather than group 2, but statistical analysis did not show any significant differences between them (P >0.05). Conclusion: Based on the obtained results in this study, it can be concluded that using succinylcholine or magnesium sulfate in RSI process has same effect on intubation outcome.

DNAJB11 (ERDJ3) Expression is a Novel Risk Factor for Breast Cancer Survival

Show Abstract
Purpose: In this study, expression, genetic/epigenetic alterations, and biomarker potential of DNAJB11 gene were evaluated in breast cancer. Methods: DNAJB11 mRNA levels were analyzed in breast cell lines by qRT-PCR. In-silico tools (UALCAN and Kaplan-Meier plotter) were used to analyze the transcript level, prognostic survival value, and promoter methylation status of DNAJB11 in clinical breast cancer samples. Methylation status of DNAJB11 promoter region in cell lines was analyzed by Methylation Specific PCR (MSP). Results: DNAJB11 mRNA expression is significantly high in breast cancer cell lines compared to the non-tumorigenic breast cell line (MCF 10A). Clinical breast cancer samples (n = 1097) have significantly high levels of DNAJB11 transcripts compared to normal samples (n = 114) (P = 1.62e-12). High DNAJB11 mRNA levels were associated with poor overall survival (n = 626) (P = 0.042), relapse-free survival (n = 1764) (P = 1.3e-09) and distant metastasis free-survival (n = 664) (P = 0.00025) in breast cancer. Genetic alterations of DNAJB11 are infrequent in breast cancer samples. DNAJB11 promoter region was frequently hypomethylated in breast cell lines and breast cancer patient samples. Conclusion: It could be suggested that epigenetic mechanisms other than DNA methylation could be involved in the DNAJB11 regulation. DNAJB11 has unfavorable survival prognostic value in breast cancer.

Non-association between rs17568 OX40 Gene Polymorphism and Type-2 Diabetes Mellitus, West-south of Iran Hot!

Background: Different environmental and genetic factors are etiologic factor for type 2 diabetes mellitus (T2DM). The identification of genes with potential for developing T2DM is necessary to prevent the onset of this disease. The aim of this study was to determine the association between OX40 gene polymorphisms (rs17568) and T2DM, west-south of Iran. Methods: This case-control study was done on 200 patients referred to Diabetes Clinics and 200 healthy subjects in Jah¬rom City. OX40 gene rs17568 polymorphism were then geno¬typed using the polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) method and biochemical parameters were measured using bio-chemical methods. Finally, the results of the study were analyzed by SPSS-19 software. Results: There were not seen significant differences between the mean of age and BMI in the case and the control groups. The analysis of rs17568 (A/G) polymorphism revealed an odds ratio 1.545 (95%CI: 0.806-2.960; P = 0.19) for the GG genotype and 1.339 (95%CI: 0.881-2.034; P = 0.172) for the AG genotype, versus the AA genotype. A-allele frequency of rs17568 SNP was higher non-significantly in patients with T2DM, than healthy subjects (70.3 % vs. 64.8 %) (95%CI: 0.955-1.730; P = 0.097). Conclusion: The results of this study indicate that OX40 gene rs17568 SNP has not association with T2DM in the evaluated population.

Electrocardiogram Showing a High P-wave in a Lung Cancer Patient with Tip Malposition of PICC: A Case Report Hot!

Background: It is recommended that electrocardiogram (EKG) showed a high P wave change, post-operation chest X-ray (CXR) should not be needed to confirm the tip of PICC location. However, in this case, the EKG showed a high P-wave change; a followed CXR revealed the tip malposition in the superior vena cava (SVC). Methods: A power PICC was placed under the ultrasound-guided modified seldinger technique in a 55-year-old man with Lung cancer. Postoperative CXR revealed that the catheter tip was ectopic in the SVC. EKG was used for adjusting the malposition, EKG showed a P-wave elevation. However, a followed CXR revealed that the tip of the catheter was still ectopic in the SVC. We conducted the second adjustment. The amplitude change of P-wave was observed from standard to high-peak and then to a downward deflection on the leading edge of the P-wave. The catheter tip was correct in the lower third of SVC. Results: We successfully adjusted the malposition of PICC with the EKG method. Conclusion: There is a risk that we cannot recognize the PCC tip malposition in the SVC with EKG, we must observe the typical P-wave change from standard to high peak and then to biphasic.
